   Full payment for both semesters is greatly appreciated at the time of the first semester registration.
   If you wish to defer payment for the Spring Semester it will be due on or before March 1, 2009.  Please indicate if you need to purchase a book/workbook set.  Include payment.

Late Registration will be accepted - with a $25 penalty - only until October 11, 2008
The only exceptions will be for families brand-new to the area and to families with a newly adopted child.
No walk-ins will be accepted on the first day.

   Chinese Dance Performance & Education Program of Maine (CDPE), CAFAM Chinese School
   or their agents may use photos or videotapes of the above named child(ren)’s
   participation for promotional or educational purposes.

   and may participate in strenuous physical activity. I hereby give permission
   for the above named child(ren) to participate in Chinese dance activities during
    Chinese School and any CAFAM or CDPE special programs.
   I hereby agree to release, discharge, indemnify and hold CAFAM, CDPE,
   Chinese School and their representatives, consultants, and all
   persons acting under their permission or authority harmless from any
   liability claims, demand costs or damages arising out of said program
   activities which my minor child(ren) might sustain during his/her/their
   participation. Permission is hereby granted for my child(ren) to receive
   emergency medical treatment and be transported to a local hospital, if needed.
    I certify that there are no limits to my minor child(ren)'s participation
   except as stated in writing above. I have read this document carefully,
   and sign it voluntarily with full knowledge of its significance.
